Admission, Discharge Guidelines


 Source of Admission:
  1. Emergency Department
  2. Cardiac Out-patient Department
  3. Referral

 Pre-admission reminders:

1.        Prepare your personal information.

2.       Organize your health information (your medical records, medical report if any, your medication prescription and keep this along with your personal information, in an updated list so it will readily be available if necessary).

3.       For (foreigner) bring your health cards or insurance as well as any special form of pre-authorization documents.

 Admission procedure:

Admission of patient from Emergency Room

1.        Emergency Room Nurse will inform the Head Nurse or Charge Nurse of the Unit concerning impending admission.

2.       Admission request form must be signed by the ROD and it should properly mention patient's data and present condition.

3.       The bed is prepared for the patient as soon as notification is received.

4.      Emergency Room Nurse should inform any emergency equipment is needed for patient admission like ventilator of the defibrillator.

5.       We ensure that the patient's name and medical number match the identification data on the wrist and take the complete endorsement from accompanying nurse.

 Admission of a patient from Cardiac Outpatient Department

1.        Bed Manager or OPD Nurse should inform the Head Nurse or Charge Nurse of the unit concerning admission.

2.       Admission request form must be signed by the ROD and it should properly mention patient's data and present condition.

3.       Vital Signs, Weight, and ECG will be taken before the patient will be shifted to ward.

4.      We ensure the bed is prepared for the patient as soon as notification is received.

5.       We ensure that the patient's name and medical number match the identification data on the wrist.

6.      Admission process (entering necessary data) as soon as the patient will reach to cardiology ward.  

Admission of patient from  as Referral Case

 

1.        If there is any referral case from other hospitals, if on-call Cardiologist accepted the case, Bed Manager will inform the unit regarding bed reservation.  

2.       If the patient arrived in Emergency Room, we expect that ER nurse should inform the Head Nurse or Charge Nurse of the Unit concerning an accepted case.

3.       We ensure that bed is prepared for the patient as soon as notification is received.

4.      We ensure that the patient's name and the medical number will match the identification data on the wrist and take the complete endorsement from accompanying nurse from another hospital especially about acceptance form, Lab investigation result, and X-ray.

    Prince Sultan Cardiac Center is not responsible for your belongings if the loss occurs. You are strongly encouraged to deposit important personal items or valuables with the Social Workers or Manager on- duty.
