ADULT CONGENITAL HEART CLINIC:

The goal of the Adult Congenital Heart Clinic is to ensure that patients born with congenital heart disease will live long, healthy lives. This is accomplished by providing research-based, cutting-edge care by medical professionals with special expertise in the complexities of congenital heart disease.
Our physicians offer care for to adults newly diagnosed with congenital heart disease and those who were treated for it as a child.
DIAGNOSIS
To diagnose and treat adult congenital heart disease, you may need one or more of the following tests.
· Magnetic Resonance Imaging (MRI)
A non-invasive technology that provides highly accurate imaging of the heart using contrast agents and 3-dimensional imaging; an MRI lasts approximately 30 minutes.
· Echocardiogram (ECHO)
An ultrasound that measures the heart’s dimensions and shape and structures, evaluates different heart functions and identifies abnormal blood-flow patterns. Also 3-dimensional.
· Cardiopulmonary Stress Testing
A treadmill test that takes an EKG (see description below) and measures lung function while you exercise.
· Computed Tomography (CT) Angiogram
A test that uses X-rays to visualize blood flow in veins and arteries, including 3-dimensional imaging.
· Electrocardiogram (EKG)
A test that uses electrical sensing devices placed on the body to measure the heart’s electrical activity.
· Holter Monitoring
A device that records heart rhythm continuously for 24 hours.
· Event Monitoring
A small device that is used by patients for weeks or months, and records heart rhythm when the patient or caregiver pushes a button.
· 24-Hour Blood Pressure Monitoring
A small device that monitors blood pressure 24 hours a day.
· Cardiac Catheterization
A small, hollow tube is inserted in an artery or vein in the groin and moved forward through the aorta. The catheter can measure pressure within the heart’s chambers, measure blockages, or obtain a sample of heart tissue for biopsy.
TREATMENT:
Your medical team will work with you to develop an individual treatment plan based on your medical needs. Treatment options might include:
· Medication Management
· Congenital Cardiac Intervention
· Stent
A device inserted permanently into an artery to keep it open and provide better blood flow.
· Atrial Septal Defect Occlusion
A catheter is used to insert a small disk-like device, which is positioned across an atrial septal defect and then allowed to expand, closing the hole.
· Balloon Dilatation of pulmonary or mitral valve
A balloon is used to dilate the aortic valve opening.
· Surgery
Patients who undergo surgery as part of their treatment may receive cardiac rehabilitation for appropriate conditioning treatments after their operation.

ARRHYTHMIA CLINIC
This clinic provides assessment of cardiomyopathy clients for consideration of:
· Automated Implantable Cardio Defibrillators (AICD)
· Bi-Ventricular Pacemakers for Cardio Resynchronization therapy (CRT)
The clinic provides assessment, teaching and follow up for various arrhythmias (e.g. Atrial Fibrillation, Supraventricular Tachycardia (SVT), Wolff Parkinson White (WPW), Ventricular tachycardia (Vtach)) and procedures (EPS/Ablation, AICD, CRT, Cardioversion, Tilt Table Testing).
SERVICE HOURS:
· Every Sunday 1:00 in the afternoon until 4:30 in the afternoon
APPOINTMENT:
· Referral from physician is required.
PACEMAKER CLINIC
As a service to our patients who have pacemakers, we provide a weekly pacemaker/ICD clinic by scheduled appointment. During these visits, the pacemaker's functions and battery level are checked. If needed, pacemaker's settings can also be adjusted. This helps fine-tune the signals so the pacemaker is always doing its best job.
The clinic works closely with your cardiologist or electrophysiologist to prolong the battery life of your device, make adjustments to improve your quality of life, and provide you with the best care.

HEART FAILURE CLINIC
At the Heart Failure Clinic we work to any patient to improve quality of life by helping you manage your symptoms of heart failure. We educate you about the condition and the ways it can be controlled. Our primary objective is to decrease the number of unnecessary visits to the emergency room, prevent hospitalizations, and help you to live a full and productive life.
The Heart Failure Clinic helps with:
§ Managing symptoms of heart failure
§ Assistance with risk factor modification, such as decreasing cholesterol, decreasing blood pressure, and improving diet
§ Managing medications
How the clinic works
To qualify for the heart failure clinic we need a formal referral from your physician. This physician may be your primary care provider, cardiologist, or internist. While progressing through the clinic, your referring physician will receive periodic reports about your progress.
During your time at the clinic you will learn the signs and symptoms of heart failure and how to deal with them..
At your initial visit you will be assigned a level of care. This level determines how many visits you are eligible for. These visits may vary based on your individual needs. and are not driven by insurance companies.
Your initial visit will last for approximately 90 minutes and includes:
§ Baseline data collection (History and Physical)
§ Weight recording
§ Diagnostic testing (This test is noninvasive and determines how your heart is functioning)
§ Education
We encourage patients to have a loved one accompany him/her to this lengthy visit for support and to help in the education process. Loved ones are more than welcome to attend each visit if necessary.
After your initial visit, typical visits will last about 30 minutes and include:
§ Weight recording
§ Medication adjustments
§ Basic assessments
